Should i go back to the doctor if they did not find anything last time this happened?

i went to the doctor a year ago with the same problem and they did a full cardiac workup and did not find anything out of the ordinary with the exeption of extreme fluctuations in blood pressure(reason unkown). the last couple of days ive had trouble breathing, chest pain and a blood pressure that goes through the roof and then down to the floor. right now i feel dizzy and i have radiating pain from my left arm, to my back and my jaw. it really hurts every time i take a breath. i know the obvious answer is yes, go! im just sick of being told nothing is wrong, or im just out for attention. im 18 btw
